Sports Physicals

Sports Exam Information

According to IHSA rules, ALL athletes must have pre-participation physicals before practicing for or participating in any sport.

The School Health Department recommends having the State of Illinois 'certificate of child health exam' form completed by your doctor for sports exams. Decatur Memorial Hospital Sports Medicine Staff conduct sports exam clinics for middle and high school students. Call your child's school for dates and times.

NOTE: The Sports Physical does NOT meet the requirements for the 6th grade or 9th grade school physical.

(A school physical does, however, meet the requirements for the sports physical.)
